Vaccine card/record. You can get a copy at https://waverify.doh.wa.gov.
Note: if under the age of 19, an ID or insurance card is not required.

Blood Glucose Screening, Blood Pressure Screening, BMI Screening, Telehealth, Vaccine Education, A1C Testing, Mental Health Screening, Vision Testing, Naloxone (Narcan) Distribution, Covid/Flu Test Kits, Local Health Insurance Enrollment Specialist, Dental Screening
